CHIP enrollment

Here is some info about CHIP enrollment that the Stake sent us. 



CHIP/Children’s Medicaid Application Drive


Saturday, January 23

10 a.m.-2 p.m.

Arlington ISD enrollment locations:

Workman Junior High

701 E. Arbrook Blvd.

Bowie High School

2101 Highbank Dr.

Webb Elementary

1200 N. Cooper St.

Knox Elementary

2315 Stonegate St.

Tarrant County families with uninsured children may be eligible for health coverage through the Children’s Health Insurance Program (CHIP) or Children’s Medicaid.

Benefits include choice of doctors, regular check-ups, prescriptions, dental services, shots, eye exams and glasses.

YOU MUST BRING:

1. a copy of birth certificate for each child - child must be a U.S. citizen or legal permanent resident.

2. a social security number for each child OR

3. an Alien Registration Card (I-551) or copy of front and back of each child’s card, if child is not a U.S. citizen.

4. proof of income for all family members - most recent paycheck stub or a letter from your employer stating how much you are paid; child support check stub; and childcare, disabled adult care, or child support paid receipt, where applicable.
